PLANNING AND RECOVERY BASED MODEL
Care Programme Approach (CPA)
Curate is committed to the development of services, which are used by patients to support their recovery and rehabilitation.
All patients have individualised treatment plans based on the recovery model, assessed need, personal goals and aspirations. The involvement of patients is an integral part of this process; patients are encouraged to work closely with the Multi Disciplinary Team in setting their objectives.
The Treatment Plan Includes the Following Elements:
- A Comprehensive Assessment of the Patient’s Health and Social Needs
- Development of Treatment Plan, based on the Recovery Model
- The Appointment of an External Advocacy Representative to Support the Patients in the Development and Implementation of their Plan
- Multi Disciplinary Team Review Process
- Development of the Treatment Plan to Encompass Review Findings
For Patients who are detained under the Mental Health Act (1983) all care and treatment plans are reviewed and delivered within the framework of Enhanced Care Programme Approach.
